To add a note, open Add records and choose Note. This is a fictional example.

Enter the event date, add a time if you know it, and choose a category. Give the note a short, factual title.

Write what happened in your own words. Separate what you observed from what someone told you. Tags can help you find it later.

Choose whether to include the note in selected reports, then select Save note.

Review the saved entry. Use Edit if you need to correct or add details. Your judgment stays at the center.
